Confidential patients' records dumped29th January 2013
A bag filled with confidential information about patients was left on top of a shredder for four days at the University Hospital of Wales in Cardiff.
A hospital employee contacted the BBC to expose the hospital's information disposal failure. Dr Graham Shortland, medical director for Cardiff and Vale University Health Board, said: "The health board places the utmost importance on patient confidentiality and takes its role as guardian of personal information very seriously.
