'Starved to death' in hospital

An inquiry has condemned the treatment of a patient who was left to starve to death in a health service hospital. 

Martin Ryan, who could not swallow because of a stroke, was not given a feeding tube due to a "total breakdown in communication". His case is one of a number which are currently being investigated by the Parliamentary and Health Service Ombudsman.
